What is Dementia Care Partners Support?

The Dementia Care Partners Support Program is a community-centered initiative designed to empower, educate, and support those caring for individuals living with Alzheimer’s disease or other forms of dementia. Recognizing the vital role care partners play, this program fosters resilience, reduces isolation, and promotes well-being through peer connection, practical education, and inclusive community engagement.

Program Goals:

    • Reduce stress and isolation often experienced by care partners
    • Provide knowledge and practical tools to empower care partners in their journey

    • Create safe, joyful opportunities for meaningful connection between care partners and those living with dementia

How We Achieve These Goals:

We offer key programs to support care partners and help build a community that understands and embraces people living with dementia.
